Supported Living Services (SLS) program focuses on training and supporting individuals with developmental disabilities by providing the necessary skills to live independently in an inclusive and supportive environment. ICS will work with the individual, in conjunction with the family and Service Coordinator to promote, develop and ensure the best environment and welfare of the individual.

About Supported Living Services

Supported Living Services (SLS) will train and support the individual with all the necessary daily living skills in the individuals's natural community and home environment. Some skills include, cooking, cleaning/organizing, budgeting and money management and sustaining a healthy living environment.

Having an established and comfortable environment enables the individual to feel more secure, at ease, and positive about the individuals ability to accomplish familiar and unfamiliar activities and goals. ICS will support the 

individual in making choices that more effectively reflect the individuals desired goals and overall well being.

Entrance Criteria

Supported Living Services (SLS) is provided to an individual that meets the following criteria.

  • Male or female, ages 18-59 yrs with an ambulatory or non-ambulatory status. 60+ years of age must have Regional Center prior approval.

  • Must have diagnosis of a Mild to Moderate developmental disability per Regional Center's guidelines.

  • Individual must require supervisor and assistance for behavioral excesses as well as for moderate deficits in self-help skills and adaptive functioning.

  • ICS will consider accepting individuals who have medical conditions that do NOT require a 24 hour nursing care, transition from a State Developmental Centers or a locked facitily to a Supported Living Environment. ICS shall review the individuals circumstances with the Regional Center on a case-by-case basis.

The Referral/Intake Process

The intake process is a collaboration between the potential individual, regional center, family members and any other members of the ID team and Inclusion Services. To begin services the following must occur.
